You are not eligible for a book loan at this time.

Terms:

•  This loan cannot be used until the next business day after it is issued to allow time for the bookstore to receive the loan information.

•  THIS LOAN MUST BE USED BY Feb 8th, 2019 by 6pm.

•  Adjustments to your account for unused portions of this loan WILL NOT be processed until the end of October. Please do not ask us for an adjustment earlier.

•  Any book purchased with this loan and returned to the bookstore is considered an adjustment. (See #3 above for processing dates) In March, any credit for returned books will have the credit applied back to your student account and will be applied to any outstanding balance for the current semester. Any remaining amount will be refunded through normal student refund processes.

•  There is a $5 loan set up fee that will be added to the loan amount.

•  Five percent (5%) simple interest will be charged from beginning date of loan until it is paid in full.

•  This loan is due on . The loan can be paid at any time, but must be paid before registration for future semesters will be allowed.

Emergency Book Loan Promissory Note

FOR VALUE RECEIVED, on 5/19/202411:45:55 AM.

I, , hereinafter called the Maker, promise to pay

the University of Houston-Downtown, hereinafter called the Institution, located at One Main Street, Houston, Harris County, Texas, the sum of $ plus interest at the rate of five percent (5%) per annum, from the date above until this loan is paid in full. Please read additional information on this form.

PLACE OF PAYMENT

All payments shall be made to the Cashier's Office, University of Houston-Downtown, One Main Street, Houston, Texas 77002. Payment may be made in person, by mail or online at www.uhd.edu .

INCIDENTAL FEE

The maker agrees to pay an incidental fee of five and no/100 dollars ($5.00) to offset the cost of processing this loan.

PREPAYMENT

This note plus accrued interest may be prepaid in part or in full at any time without penalty.

WAIVER OF PROTEST

The maker of this note and any co-maker, sureties, guarantors, heirs and legal representatives waive demand, grace, notice, presentment for payment, protest and notice of protest, and diligence in collecting or bringing suit.

ACCELERATION

The failure of the maker to pay any principal or interest as it becomes due gives the institution the right to mature the same and declare the entire balance due and payable forthwith.

WAIVER

No delay or omission on the part of the institution in exercising any right hereunder shall operate as a waiver of such right or of any other right under this note. A waiver on any one occasion shall not be construed as a bar to or waiver of any right or remedy on any future occasion.

COLLECTION COSTS

In the event this note is placed in the hands of an attorney or collection agency for collection, the maker agrees to pay all fees and costs incurred by the institution in enforcing the terms of this note.

CHANGE OF NAME AND /OR ADDRESS

The maker agrees to immediately inform the institution Office of Records of any changes in name and/or address.

PENALTIES ON DEFAULT

Failure of the maker to abide by the terms and conditions of this note may result in any or all of the following:

•  The institution may withhold the maker's transcript(s) and diploma;

•  The maker may be barred from further enrollment at any component institution of the University of Houston System unless and until the maker satisfies his outstanding indebtedness;

•  The outstanding loan amount may be charged to the maker's student account, all institutional services may be severed, and the maker shall be charged a fee of fifteen and no/100 ($15) to restore such services ;

•  The institution may refer any defaulted account to a collection agency and a collection fee of no less than 25% (1/4) of the outstanding loan balance shall be added to the maker's student account at the time of referral.

PAYMENT WITH FINANCIAL AID FUNDS

I understand that if I receive financial aid (loans, grants, scholarships) during the semester in which this loan was obtained, financial aid funds will be used for repayment of the loans. I hereby authorize the University of Houston-Downtown to apply all financial aid funds to repay my institutional loan(s) and any additional financial aid funds will be paid to me directly. In the event that financial aid funds are insufficient to cover repayment of this loan, I will be responsible for the remaining indebtedness.

DROPPED CLASSES

I understand that if I drop any or all classes, any refund of tuition and fees will be first applied to the remaining balance of my account, including all loans, before any monies will be refunded to me. I also understand that if the refund from the dropped class(es) is less than the remaining balance of my account, I will be liable for the difference.
